Health Home Care Management

Our Health Home Care Management team partners with clients and, as appropriate, their caregivers to ensure that they receive everything they need to stay healthy and out of the hospital. The nature of support provided is determined by individual goals and needs.

PCHO prioritizes safe and stable housing as a tool towards helping individuals and families successfully engage with the healthcare system.

Additionally, our trained care managers work with approximately 175 individuals every month to promote health in a variety of ways including:

Ensuring that clients have access to healthy food;

Providing healthy opportunities for community engagement;

Connecting individuals to necessary social supports that promote wellness; and

Supporting transportation needs that make care providers more accessible.
